The action supports the creation of special care centers (including the construction of family homes) for people with special mental needs in order to provide better living, learning and working conditions. In the places created, a person can live as normal a life as possible.

EUR 47.6 million from the European Regional Development Fund (ERDF) has been earmarked for the modernization of special needs institutions, with additional funding of at least EUR 8.4 million. By 2023, it is planned to reduce the number of large dormitory-type care homes and replace them with smaller facilities with up to 30 beds. It was planned to create at least 1,400 high quality service outlets, including 1,200, and to support the creation of 200 new community service outlets. The implementing body for the action is the National Support Service.

On January 12, 2017, the Government of the Republic approved the Investment Plan, which has a list of 10 projects related to the reorganization of existing large institutions and 10 projects for the creation of new community service places. A total of €35.8 million is planned for the implementation of 20 projects, plus €4.5 million in self-financing and € 3.6 million in national co-financing. The scheme will create a total of 1312 service locations, including 992 reorganizations and 320 new 24-hour community, community and assisted living services.
